Development and Implementation of a Digital Optimization Toolkit for Management of Heart Failure (DOT-HF) after Discharge. This project is developing a multi-component intervention to improve the up-titration of guideline directed medical therapy for systolic heart failure, including a patient facing mobile phone app, remote patient monitoring, and a clinician dashboard. UW is partnering with the Federal University of Minas Gerais to adapt the intervention to the Brazil context.
